WRAP WILL HELP YOU

• Discover your own simple sateness wellness tools

• Develop of list of things to do every day to stay as well as possible

• identify upsetting events, early warning signs, and signs that things have gotten much worse and, using wellness tools, develop action plans for responding

PRP CRITERIA:

• Adults 18+ years of age

• Must meet priority population diagnosis criteria

• Desire to participate in program and services on a voluntary basis

• Must Have Maryland Medicaid

• Self Pay is available

• Live within proximity of our office or can self transport or use public transportation

• Individual is not in a current need of a higher level of care such as inpatient care, complex medical issues, active substance abuse

PRP offer rehabilitation services in both day program and community based/ outreach setting
